Mary Washington Health Alliance is physician led, physician governed integrated network of 400 physicians covering more than 80,000 lives throughout the Rappahannock region of Virginia, between Richmond and Washington D.C. The broad reach of physician leadership and governance has important implications on how effective Population Health Management is adopted successfully. These Population Health management strategies include incorporating evidence-based information more quickly into patient diagnosis and treatment, engaging patients and families more directly in care, delivering higher levels of team-based care, and providing more-transparent cost and quality data to physicians and other providers. As the country moves away from fundamental fee-for-service payments, successful ‘interdependence between hospitals and physicians becomes necessary in defining a clinically integrated delivery system that gets reimbursed for keeping the community healthy. MWHA has built on Mary Washington Healthcares (MWHC) rich 100 year history through partnerships with key stakeholders who share a common mission of improving the health of the population they serve. After completing (3) consecutive years in the Medicare Shared Savings Program and having shown savings and improved quality of care, the Alliance is ready to take on the road ahead within the Next Generation ACO! As 1 of 51 participants in the Next Generation ACO Model, MWHA will be able to build on the current level of coordinated care and will provide an enhanced quality of care at lower costs to Medicare patients.
