Inspira Health Network is a charitable nonprofit health care organization serving communities across southern New Jersey. The network, which traces its roots to 1899, comprises three hospitals, a comprehensive cancer center, several multi-specialty health centers and a total of more than 100 locations. These include urgent care; outpatient imaging and rehabilitation; numerous specialty centers, including sleep medicine, cardiac testing, digestive health and wound care; hospice and home care; and more than 30 primary and specialty physician practices in Gloucester, Cumberland, Salem and Camden counties. Together with our medical staff of more than 1,100 physicians and other health care providers, we provide evidence-based care to help each patient achieve the best possible outcome. Our clinical and support staffs are focused on providing safe care in a safe environment for every patient. Approximately 70,000 skilled nursing facility (SNF) residents in the United States are diagnosed with end-stage renal disease (ESRD). Providing effective dialysis care to nursing home residents presents unique challenges for operators. The SNF population is highly decentralized, presenting various logistical challenges and necessitating innovation relating to technological integration, coordination of care, and clinical oversight. With Concerto`s innovative model, these challenges are overcome, resulting in an improved process and healthier outcomes for SNF dialysis patients. Our on-site home hemodialysis services are designed to add value to a nursing home and position operators as market leaders in sub-acute dialysis care.
